Your Outpatient Experience Now Has a Financial Stake. Is Your Feedback Program Keeping Up?

Outpatient Patient Experience Now Has a Financial Stake

For years, hospital patient experience programs have focused heavily on the inpatient stay.

That is no longer the whole picture.

CMS requires applicable hospital outpatient departments (HOPDs) and ambulatory surgery centers (ASCs) to conduct and submit the Outpatient and Ambulatory Surgery CAHPS (OAS CAHPS) survey as part of quality reporting. The survey measures patient experience across outpatient surgeries and procedures, extending formal measurement beyond the inpatient setting (CMS).

For hospital leaders, that creates an opportunity to extend patient experience visibility across more of the care journey.

Outpatient and ambulatory surgery operate differently from inpatient care. The timeline is shorter, the journey is more compressed, and there may be only a few hours to identify and address an experience issue before the patient goes home.

Real-time feedback can help close that window.

The Financial Stake Is Explicit

Outpatient patient experience is not simply an additional metric.

CMS states that OAS CAHPS is required and linked to reimbursement for applicable HOPDs and ASCs. Facilities that do not conduct and submit OAS CAHPS as required can receive a 2.0 percentage-point reduction in their annual payment or fee schedule update (CMS).

The Ambulatory Surgical Center Quality Reporting (ASCQR) Program similarly requires participating ASCs to meet program requirements to receive their full Medicare annual payment update (CMS).

That gives outpatient patient experience a clear financial dimension.

For health systems, the operational implication is just as important: visibility into the patient journey should not stop at the inpatient unit. Outpatient departments and ambulatory surgery centers require the same attention to patient experience, adapted to the way care is actually delivered in those settings.

Outpatient Feedback Requires a Different Approach

Extending patient experience measurement into outpatient settings does not necessarily mean replicating the inpatient model.

The outpatient journey moves faster.

A same-day surgery patient may complete preparation, check-in, a procedure, recovery, and discharge within a single visit. That creates a narrower window for teams to identify confusion, frustration, or unmet needs while staff still have an opportunity to respond.

The experiences CMS measures also reflect that different journey.

OAS CAHPS asks patients about preparation for their surgery or procedure, check-in, facility cleanliness, communication with staff, discharge, and preparation for recovery at home (CMS).

Those are highly operational moments.

Was the patient prepared for what would happen? Did staff communicate clearly? Was the environment clean? Did the patient leave understanding what to expect during recovery?

A real-time feedback program designed around those moments can give teams an earlier indication of where the experience is working and where additional attention may be needed.

Real-Time Signals Create an Opportunity to Act

Formal patient experience surveys provide an important standardized view of performance. Real-time feedback can serve a different purpose: helping teams see and respond to experience signals while care is still being delivered.

Consider a same-day surgery patient who is uncertain about discharge instructions.

If that signal reaches staff before the patient leaves, someone can clarify the instructions, answer questions, and help the patient feel better prepared for recovery.

The same principle applies to check-in, communication, cleanliness, and other parts of the outpatient journey measured through OAS CAHPS.

CMS currently reports OAS CAHPS results for both HOPDs and ASCs, with HOPD results also publicly available through Medicare's Compare tool (CMS).

Real-time feedback gives operations leaders another layer of visibility between those formal reporting periods.

Instead of learning only how patients viewed an experience after the fact, teams can identify emerging patterns as they happen and determine whether an operational response is needed.

Consistency Matters Across Care Settings

Health systems increasingly deliver care across multiple settings.

CMS itself continues to recognize the evolving role of outpatient care. In its CY 2026 outpatient payment proposals, the agency noted that advances in medical practice allow more procedures to be performed on an outpatient basis with shorter recovery times (CMS).

That makes consistency across the outpatient footprint increasingly important.

A patient may associate a hospital outpatient department and an affiliated ambulatory surgery center with the same health system brand, even when the facilities operate differently.

Their expectations do not necessarily change with the facility type.

CMS reports OAS CAHPS results for both HOPDs and ASCs, creating standardized visibility into experiences across these settings (CMS).

Health system leaders can complement that standardized measurement with continuous feedback that helps identify differences across locations, journeys, and operational touchpoints sooner.

The goal is not to replace OAS CAHPS.

It is to give teams more opportunities to understand and improve the experiences that OAS CAHPS ultimately measures.

The Bottom Line

Outpatient and ambulatory surgery are becoming a more important part of the patient experience landscape. CMS quality reporting requirements have put clear financial and operational importance behind understanding how patients experience these settings.

Extending real-time, journey-specific feedback into outpatient departments and ambulatory surgery centers gives teams visibility while there is still time to act. It connects formal measurement with day-to-day operations — helping teams understand the experience today rather than waiting to discover it later.
